  • The "'posterior surface "'is traversed by a shallow groove directed obliquely downward and medialward, continuous with a similar groove on the posterior surface of the talus and serving for the passage of the tendon of the Flexor hallucis longus.
  • The medial portion is blended with the Abductor hallucis previous to its insertion; the lateral portion with the Adductor hallucis; the tendon of the Flexor hallucis longus lies in a groove between them; the lateral portion is sometimes described as the first Interosseous plantaris.
  • Plantarflexion of the foot : Primary muscles for plantar flexion are situated in the Posterior compartment of leg, namely the superficial Gastrocnemius, Soleus and Plantaris ( only weak participation ), and the deep muscles Flexor hallucis longus, Flexor digitorum longus and Tibialis posterior.
  • An example of an intrinsic factor for tendinopathies are : poor biomechanics such as limb malalignments and hyperpronation that may cause increased traction loads acting on the foot and ankle that may increase the incidence of Achilles, flexor hallucis longus muscle, and tibialis posterior muscle tendinopathies.
  • Its upper third is rough, for the origin of the soleus; its lower part presents a triangular surface, connected to the tibia by a strong interosseous ligament; the intervening part of the surface is covered by the fibers of origin of the flexor hallucis longus.
  • It passes obliquely forward and lateralward, superficial to the deltoid ligament of the ankle-joint, into the sole of the foot, where it crosses below the tendon of the flexor hallucis longus ( plantar tendinous chiasm ), and receives from it a strong tendinous slip.
  • The sheath for Flexor hallucis longus reaches up to the level of the tip of the malleolus, while that for the Flexor digitorum longus is slightly higher; the former is continued to the base of the first metatarsal, but the latter stops opposite the first cuneiform bone.
  • Enumerated from the medial side, the four canals which it forms transmit the tendons of the tibialis posterior and flexor digitorum longus muscles; the posterior tibial vessels and tibial nerve, which run through a broad space beneath the ligament; and lastly, in a canal formed partly by the talus, the tendon of the flexor hallucis longus.
  • On its lower edge on either side are its lateral and medial processes ( serving as the origins of the talus; below, it is grooved for the tendon of the flexor hallucis longus; its anterior margin gives attachment to the plantar calcaneonavicular ligament, and its medial margin to a part of the deltoid ligament of the ankle-joint.
  • The flexor retinaculum sends fibrous septa to the bone to form four separate osseofibrous tunnels for the following structures which are from anterior to posterior : the tendon and the synovial sheath of tibialis posterior, the tendon and synovial sheath of flexor digitorum longus, the posterior tibial vessels and their tibial nerves and the tendon and synovial sheath of flexor hallucis longus.
